• When teaching important, diagnosis-related diet changes, we can go into your kitchen with you or your cook and have very specific discussions about what is best for you.
• We teach exercise programs tailored to your condition, and we teach you how to do them independently at home. This eliminates the need to go outside or to go to a gym and sets you up for greater success.

Our Home Health Professionals

Skilled Nursing
Assesses, teaches and provides patient care for a variety of conditions, including congestive heart failure, diabetic care, post-operative care, and more.

Physical Therapy
Aids with pain relief, fall prevention, regaining strength, mobility and range of motion, and more.

Speech Therapy
Improves communication skills, restores cognitive skills, and aids with swallowing problems.

Occupational Therapy
Restores self-care skills, trains in the use of self-care equipment, and activities planning.

Home Health Aides
